Club Events and Fundraisers

Many clubs hold fundraising events or other projects on campus. Here are the forms you need to successfully gain approval for your on-campus event or field trip. NOTE - these forms are required in order to have a field trip or a fundraiser/ event at Paly (they need to comply with school, district, state, and federal law). To plan a successful event, follow these 4 simple steps:

 

Step 1: Plan Out the Event Details:

Do some research before starting the paperwork process! Work with your club to have a plan ready!

Step 2: Complete Forms

CLUB PROJECT PROPOSAL FORM - This form is necessary for all "school affecting" events  -meaning they happen on campus and require facility use or student participation. Be sure to sign the agreement box.

BUDGET FORM involving the collection of money require this form.

FUNDRAISING POLICIES All approved clubs must have the agreement slip filled out.

REIMBURSEMENT FORM - Turn it in to the Auditor, located in the tower building. The auditor will provide this form soon.

Step 3: Get Approval!

All signatures on the forms must be filled out. The Student Activities Office (SAO) is located across from the Paly Main Office.

Step 4: Advertise!

POSTER REQUEST FORM - If you want help in making your posters let ASB Spirit Commissioners help you! Just fill out this form and drop it off in the SAO.
